I cannot say enough positive things about The Plaza. The staff is compassionate, attentive, and truly cares about the residents' well-being. The facility is clean, well-maintained, and provides a welcoming environment. Communication with families is excellent. Definitely would recommend to anyone.

it's not the best I've seen, but the staff goes out their way to try and accommodate you it seems like every place serves cold food, at least you have a microwave for those who can get to it. I've been to places I like a lot less who pack the lunch for patients when they go out to a doctor's appointment or whatever I thought that was a nice gesture they also provided a transport wheelchair, which is small and easy toTravel with and lift.

My mom was sent to this place on March 2 when I was told her stage 4 cancer had spread and she only had weeks to live. I was told my mom would be in hospice and she was put on the 4 floor in rehabilitation were they came and had my mom sit and stand and move around. I was totally confused when I asked questions no one had answers. Finally after about 2 weeks she was moved to hospice on the 11 floor and when we went up the nurses said another one as if she was bothered they had to work and take down information and set her up in a room. For 2 days my mother never received her pain medication she passed at 4 pm and I asked did she pass because she was no longer breathing and when I went to the nurse station to ask she said well your mother is dying you did know that and took an hour before they even came in the room to check vitals. Her oxygen machine that was plugged in that I thought was giving her oxygen was broken and they told me that when they came in an hour later they came with new oxygen at 6 pm when I knew my mother had already passed. The nurse had no answers I sat there till 10 pm and when I got home a nurse called to tell me she passed. This place is so unprofessional they will leave there patient to die. The lady next to my mother has dementia and she will scream for hours and the nurse came by one time and just told her to be quiet and never came back in the room. This place is horrible patient are not being looked after properly and there are no signs of these nurses and doctors even caring one bit of the patient well being. Please don’t leave. Your family member unattended. They just want to charge your insurance and that is it!

My 99yrs old mom was in palliative care in this facility before she passed recently. I can not Thank YOU enough, all the Doctors, Nurses, CNA, PT, Resp therapist, Social workers ..West bldg 3rd floor , even a simple eye drop or chage the dressing of little scratch in her knee were not ignored...I don'thave to request to change her diaper because they constantly checking it .The young PTs treated her as their grandmother, massaging her legs if she is sleeping or not in the mood for therapy. And to the night shift staffs who called me when she wasn 't doing well Thank you so much at least she did not expired alone. The staff offered us water and tissues to wipe our tears and hugs to ease our pain. The nurse told me that she emailed the Social worker that I will be waiting for her call to answer my question but the sw start at 9:am but I was so surprised she called at 8:15 because of the situation ..Again The Plaza. Thank you!

I met with the admin and recreation team, and they openly explained their process. They were also unaware that Citadel emails weren’t being received. I appreciated their transparency and their commitment to improving the system. I’m hopeful my concerns will be addressed moving forward. Urgent Concerns: Resident Safety, Mismanagement, and Lack of Responsiveness – 6th Floor (West)

They are not good. My foot was almost amputated due to their negligence. Some of the nurses were very attentive, however I would NEVER recommend this facility to anyone. I got COVID at this facility and I would have never known unless my sister came to visit me. If you are looking for a place for yourself, or loved one, don’t come here. NO BUENO

Clean, organized, quiet environment, loving and caring employees with Patients and visitors ... we were in six people and went to visit a friend and pray with her... foi tudo maravilhoso ... eu recomendo ❤️🌹❤️ Parabens pelo profissionalismo de todos ! Everything was wonderful... I highly recommend it. ❤️🌹❤️ Congratulations to everyone on their professionalism. !
